Parents new to the school system, as well as those returning to the district, register their children into any of the Toms River Schools from one location, our Central Registration Office at 54 Washington Street in downtown Toms River. The staff at this office will be able to answer many questions regarding the schools as well as provide information regarding necessary paperwork needed for registration. It is not necessary to bring your child. You need only bring the required documents.
Registration Requirements and Forms:
Proof of Residency
You must provide four (4) documents, as detailed in the residency checklist section of the application below, for verification of residency in the communities of either Beachwood, Pine Beach, South Toms River or Toms River, New Jersey. You will not be able to register without these documents in hand on registration day. (Please bring copies to be kept on file.)
Additionally, this document is required when an entire family will be residing with a town resident
State statute 18A:40-4 requires all students to have a physical examination by a physician. See below to download the Physical Examination record and bring it to your physician. Your physician will complete this form upon the conclusion of your child's physical. You may still register now if your child has not completed all needed examinations and inoculations. This completed physical examination report must be signed by the physician and submitted to the school within 30 days of initial entrance.

Bring a copy of your child's immunization records consisting of dates of Primary Series and booster doses. N.J.S.S.C. Chapter 14 requires that immunizations be complete and up-to-date otherwise, the student may be excluded from school. If completed, please bring immunization forms with you to your in-person registration.
The doctor's signature must be on the immunization records. If your child was not immunized against mumps and/or regular measles because he/she had the disease, documentation signed by a physician is required. Children will not be assigned to a class in September until all records are complete. (please bring a copy for us to keep on file.)
Students without the appropriate documentation will be allowed to enter school under provisional admission as long as a completed physical, updated vaccine documentation, and/or appointment date for medical provider takes place by August 15, 2015. Documentation must be received prior to the start of school or your child may be excluded from school.
Settlement Agreement and/or Court Orders (if applicable)
This requirement only applies in situations regarding parental rights, limitations due to divorce or separation.
Affidavit of Residency (if applicable)
This requirement only applies in situations where student is living in Toms River due to emergency or hardship. This Affidavit must be signed and notarized. Please see below to download the appropriate Affidavit of Residency form.
Other Optional Forms:
* Babysitting providers need only provide one primary Proof of Residency from the top section of the Residency Checklist linked above.
