KINDERGARTEN REGISTRATION FOR
2010-2011 SCHOOL YEAR!
Students must be age 5 on or before October 1, 2010 to enter
kindergarten in September
- NO EXCEPTIONS!
We are now accepting Kindergarten
Registration for the 2010-2011 school year. In an effort
to make the registration process easier, you may now register
online by accessing the
Registration Form from our website @
www.trschools.com.
Please open the form, complete using the tab key to maneuver
through the document and save it to your computer. Once
saved, please attach the form to an email and send it to
centralregistration@trschools.com.
Your registration folder will be waiting for you at the Central
Registration office located at 54 Washington Street in downtown
Toms River. For those who are unable to use the on-line
process, you may appear in person at our Central Registration
Office. Please refer to our website to download forms, or
you may pick up a registration packet at your nearest elementary
school.
Our office will finalize or process registrations Monday through
Thursday, from 9:00 a.m. to 2:00 p.m. It is not necessary
to bring your child. You need only bring the required
documents listed below during the designated dates shown below.*
Your child will not be able to
begin school without this documentation.
Proof or residency: Tax bill, Deed or Mortgage statement
for homeowners; lease for renters and current
utility bill. For persons residing with someone else who
owns or rents, the above items must be provided by the homeowner
or renter and a
Third Party Residency Form must be completed and notarized.
The parent/guardian will also be required to provide a regular
monthly mailing in their name (i.e. cell phone bill, bank
statement, etc.)
Original Birth Certificate for the student.
Photo ID for the parent registering the student. Custody
papers may be required to prove residential custody for the
student. Please note: Only the natural parent
or legal guardian may register the child.
Health records: (Click here to download the medical
forms required, including the physical form.)
a. Complete physical (signed, stamped & dated September 1,
2009 or later by the physician.)
b. Completed immunizations as listed below:
DTP/DT: 4 doses w/1 dose on or
after the 4th birthday or 5 doses
Polio:
3 doses w/2 dose on or after the 4th birthday or 4 doses
MMR:
2 doses w/1 dose on or after 1st birthday
Hep B: 3
doses
Varivax: 1 dose on
or after the 1st birthday
Above documentation must be brought to the Central
Registration Office as soon as possible.
