Requirements, Dates and Forms
As a result of a state mandate, requirements for 2007-08 Sports Physicals have changed.
Students must have their physicals performed at their own "medical home" (family
physician/advanced practice nurse). If a student does not have their own "medical home",
the certified school nurse will offer information to the parents/guardians on current New
Jersey sponsored health insurance programs (see the below Notice of Change for more details).
Please note the following important information about sports physicals:
- All physicals must be completed using the forms provided by the school (Health History,
to be completed by parent/guardian and Pre-Participation Physical Examination, to
be completed by physician/health care provider). NO OTHER FORMS WILL BE
ACCEPTED.
- Physical exams may not be done by a chiropractor.
- Sports physicals must have been completed within 365 days of the first day of tryouts for
any given sport.
- All physicals must be reviewed by the school physician, per regulation. Therefore, they
must be submitted by the assigned school deadline.
