
2007-08 SPORTS PHYSICALS Requirements, Dates and Forms
As a result of a state mandate, requirements for 2007-08 Sports Physicals have changed. Students must have their physicals performed at their own “medical home” (family physician/advanced practice nurse). If a student does not have their own "medical home", the certified school nurse will offer information to the parents/guardians on current New Jersey sponsored health insurance programs (see the below Notice of Change for more details). Please note the following important information about sports physicals: All physicals must be completed using the forms provided by the school (Health History, to be completed by parent/guardian and Pre-Participation Physical Examination, to be completed by physician/health care provider). NO OTHER FORMS WILL BE ACCEPTED. Physical exams may not be done by a chiropractor. Sports physicals must have been completed within 365 days of the first day of tryouts for any given sport. All physicals must be reviewed by the school physician, per regulation. Therefore, they must be submitted by the assigned school deadline. WINTER PHYSICALS Saturday, October 27, 2007 At the HSE Cafetorium 8 AM - 12 Noon SPRING PHYSICALS Saturday, February 2, 2008 At the Poland Spring Arena 8 AM - 12 Noon
