If your child is interested in trying out for any sport, including intramurals and cheerleading at Intermediate School, he/she is required to have a sports physical to do so. You can download a copy from the Toms River School website, just copy and paste the line below. If your child is planning to try out for a fall sport ( Soccer, Field Hockey, Cross Country, Volleyball, Cheerleading and Intramurals ), he/she must turn in a sports medical packet to the main office at least two weeks prior to the first day of school. There will be a log in sheet when your drop off your packet. Please note, you may drop off the packet for the 17/18 school year to your Intermediate School beginning the last week in June. Try outs at the Intermediate level usually begin the second week of school, and no student will be able to try out without a cleared physical. All Sports physicals must be cleared by the school physician prior to try outs, which can take up to two weeks. Also there have been new state regulations and mandates in regards to the physical forms. Only physicals that are signed, stamped and contain the cardiac assessment development module physician assigned number will be considered complete and valid. Ø Please note that the DATE OF THE EXAM ON A PHYSICAL is only good for one year from the original date. It does not go by the date that you hand it in to the school. Ø HEALTH HISTORY UPDATE QUESTIONNAIRE form will need to be completed for every new sport a student who has a cleared physical tries out for or if a sport physical was handed in and a sport was not played within 90 days.
